Error code

AD3 — Not allowed with visit

Not payable with a visit

This service can't be paid together with the visit billed on the same claim; the Schedule treats it as part of the visit.

What to do: Keep the code the payment rules allow, remove the other, and resubmit.

Code
AD3
Description
Not allowed with visit

Explanation

Updated October 26, 2026

This service can't be paid together with the visit billed on the same claim. Remove one or bill the right combination.

What it means

AD3 means the service is not allowed with a visit. The Schedule treats it as part of the visit, or as not payable with one, so the claim was rejected.

On the Remittance Advice, the same bundling rule shows as code D3.

Common causes

  • A service billed with an assessment when the Schedule includes it in the visit
  • A procedure that can't be billed with a visit on the same day
  • Two codes billed for the same encounter when only one is payable

How to fix it

  1. Check the Schedule of Benefits payment rules for the codes billed together.
  2. Keep the code the rules allow and remove the other, or bill the procedure alone if that's what the rules say.
  3. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process described under VJ7.

Don't resubmit if

  • You haven't changed the combination of codes.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
