Explanation

Updated October 26, 2026

Repeat chemodenervation (phenol, alcohol or similar) of a peripheral motor nerve for focal spasticity within 30 days of a previous injection: first nerve.

What it covers

Chemodenervation is the injection of an individual peripheral motor nerve with phenol, ethyl alcohol or a similar non-anaesthetic chemical agent to reduce focal spasticity, and may include EMG guidance of the injection(s) (J105).

G487 is the repeat or additional procedure within 30 days of a previous chemodenervation injection, for the first major nerve and/or its branches; each additional major nerve the same day is G488. The initial procedure codes are G485 and G486 (J105).

Billing rules

  • G487 applies only to a repeat or additional procedure within 30 days of a previous chemodenervation injection (J105).
  • If an anaesthetic agent is used instead of phenol, alcohol or a similar agent, bill the nerve block listings instead (J105).
  • Chemodenervation into the same muscle on the same day as a botulinum toxin injection is insured but paid at nil (J105).
  • The listing includes EMG guidance of the injection(s) when it is used (J105).
  • Payable in addition to a consultation or assessment the same day (J3).

Often billed with

  • G488 for each additional major nerve and/or its branches the same day (J105).
