Payable only with G875 (J51).
Maximum of 11 additional injections (J51).
When EMG and/or ultrasound guidance is needed for two or more injections, add G878 (J51).
Limited to one treatment per condition per patient every 10 weeks. If more frequent treatment is needed, submit the claim for manual review with supporting documentation (J52).
When used to find the injection site, EMG or ultrasound services other than G877 or G878 are not payable with botulinum toxin services (J52).
