Explanation

Updated October 26, 2026

Weekly all-inclusive physician team fee for a patient on chronic haemodialysis at an ICHSC, paid to the most responsible physician.

What it covers

The chronic dialysis team fee is the all-inclusive weekly benefit per patient for the professional side of managing chronic dialysis and end-stage renal failure. It is modality independent and has the same value whether dialysis is in hospital, community or home, haemodialysis or peritoneal (J39).

G863 is the weekly team fee when the predominant modality and location is haemodialysis in an Integrated Community Health Services Centre (ICHSC) (J40).

Billing rules

  • One weekly fee per patient for a 7-day period starting at midnight Sunday, payable to the most responsible physician; other physicians' services that the fee includes are paid at nil (J39).
  • Claim the code for the predominant location and modality that week (J40).
  • If the patient is treated part of the week at an auxiliary treatment centre, the principal centre's claim is reduced to 1/7 of the weekly fee for each day the patient was its responsibility (J39).
  • Includes all consultations and visits to manage chronic dialysis (including in-patients on chronic dialysis), nephrology and internal medicine care of dialysis complications and end-stage renal disease, related counselling, interviews, psychotherapy and case conferences (J39).
  • Not included, so billed separately: ED assessments and special visit premiums for emergent calls; admission assessments and subsequent visits for acute in-patients with complications or intercurrent illness; other procedures including acute dialysis; consults by other specialties; the family physician's primary care; transplant program entry assessment (J39).
  • Marked #: the fee doesn't include premises, equipment, supplies or staff (J1).

Often billed with

  • Hospital admission assessment and subsequent visits (for example C162) when the patient is admitted for complications or intercurrent illness, which the team fee excludes (J39).
