Point-of-care ultrasound by the ED physician for suspected tamponade, cardiac standstill, traumatic intraperitoneal bleeding, ruptured AAA or ectopic.
What it covers
H100 is an ED investigative ultrasound personally done by an ED physician who meets training and experience standards, when no diagnostic radiologist is available for an urgent interpretation (A17).
The patient must be clinically suspected of pericardial tamponade, cardiac standstill, intraperitoneal hemorrhage from trauma, ruptured abdominal aortic aneurysm, or ruptured ectopic pregnancy (A17).
Billing rules
- Maximum 2 per patient per day, the second only as a follow-up for the same condition. Submit more than 2 with the manual review indicator and supporting documents (A17).
- When H100 images are payable, Diagnostic Ultrasound section codes (technical and professional) are not payable to any physician (A17).
- Other than H100 and J149 guidance, ultrasound codes are not payable to ED physicians (A16).
- Chart a permanent record of the images and an interpretive report (A17).
- Equipment must allow another trained physician to reach the same interpretation, have B- and M-mode, and a trans-abdominal probe of at least 3.5 MHz (A17).
- Not payable fee-for-service in an ED covered by an alternative funding agreement, except under the AFA's second on-call or GP Expert group numbers (GP110).
Often billed with
- The ED assessment for the same visit, e.g. H102 or H122 (A16, A17).
Common rejections
- ESD: The hospital ED is part of an alternative funding agreement, so H-prefix services there aren't paid fee-for-service unless billed under the AFA's second on-call or GP Expert group number (GP110).
- AM1: Service limit exceeded: more than 2 H100 for the patient that day; resubmit with the manual review indicator and documentation (A17).
