K887 covers CTO initiation, including completing the CTO form and all preceding CTO services directly related to initiation (A27).
They include all related visits, interviews about rights advice and consent, preparing the plan, case conferences, calls and messages, and CTO forms such as Form 45 and Form 47 (A26).
Billing rules
- One all-inclusive initiation claim per patient per physician per initiation in any six-month period (A27).
- Submit on an Independent Consideration basis with the Ministry's elapsed-time form, and keep a copy in the chart; otherwise the service pays nil (A27).
- Units: add the actual minutes of each insured activity, divide by 30 and round to the nearest whole unit (A27).
- Bill only services you personally rendered; services by others pay nil (A26).
- Travel time units are payable at nil; travel time and expenses for Consent and Capacity Board appearances are not insured (A27).
- Separately billable: emergency ED or in-patient assessments and special visits, Form 1/Form 3 work, unrelated conditions, and in-patient services not tied to initiating the CTO (A26).
- Can be done by video or phone with an existing patient relationship; add K300A or K301A (AJ1, A66).
