Caesarean section with tubal interruption at the same operation. The sterilization is part of P041, not billed as a separate procedure.
What it covers
P041 is caesarean section including tubal interruption (K6). It is the listing to use when the patient has a tubal sterilization at the caesarean; the tubal interruption is included in the listing rather than claimed as a separate procedure such as S741 (K6).
Like the other labour-delivery codes it carries the specific elements of a # independent operative procedure, so the fee does not include the operating room premises, equipment and staff (K1, SP1). The surgeon bills suffix A, the assistant B and the anaesthesiologist C; the assistant's fee is 6 basic units and the anaesthesia fee 7 basic units, plus time units (K3, K6, GP85-GP86, GP92-GP93).
Billing rules
- Multiple births: one of P018, P041 or P042 is paid, plus E499 for the second caesarean delivery and E500 for each later baby; if the first baby is born vaginally, add 85% of P006 or P020 (K6).
- Between 20 and 23 weeks' gestation, only one of P018, P041 or P042 plus E499 is payable; E500 is not (K6).
- Ordinary immediate care of the newborn is included; newborn resuscitation is billed as G521-G523 (K3).
- Surgeon: E411 sole delivery premium is payable with P041A when it is the only delivery that calendar day, maximum 25 per physician per fiscal year (K7).
- E676 morbid obesity premium may be added (BMI over 40 and charted; general anaesthesia, which in the Schedule means any anaesthesia other than topical or local infiltration, so a spinal or epidural qualifies (GP3)), once per patient per physician; P041 is on the eligible list (SP6-SP7).
- A physician claiming P009 for the same patient cannot also claim assistant or anaesthesia units (K7).
- FHO: P041A counts toward the labour and delivery special payment (Agreement p.66 §8.4).
Often billed with
- E499 and E500 for the second and later babies (K6).
- E400B/E401B assistant after-hours premiums when the case starts after hours (GP87).
- E676A/E676B for a morbidly obese patient under general anaesthesia (SP6).
