What it covers

S745 is an oophorectomy and/or oophorocystectomy (V10). With total omentectomy, bill S782 instead (V10).

Ovarian excision codes cover unilateral or bilateral surgery (V10, note 2).

Billing rules

• Add E090 only when the contralateral ovary has moderate or severe endometriosis and is removed (V10).
• Not payable with hysterectomy S757, S816, S758, S759, S710, S763 or S762 (V8, note).
• No claim for a simple ovarian or para-ovarian cyst removed incidentally during another operation (SP2, §3).
• More than one procedure under the same anaesthetic (or within 14 days in the same admission for the same condition): the major procedure is paid in full and the others at 85% (SP2, §3).
• The surgical fee does not include the major pre-operative visit (where the decision to operate is made), which is billed separately; the surgeon can't bill the hospital admission assessment unless it is that visit (SP2, §1).
• Eligible for the E676 morbid obesity premium: BMI over 40 charted, general anaesthesia (SP6, SP7).
