Add-on for removing ovaries with moderate or severe endometriosis with hysterectomy S757, S758 or S759, or the other ovary with S745.

What it covers

With hysterectomy S757, S758 or S759, E090 pays for removal of one or both ovaries with moderate or severe endometriosis (V8).

With oophorectomy S745, E090 pays for removal of the contralateral ovary with moderate or severe endometriosis (V10).

Billing rules

• Only with S757, S758, S759 or S745 (V8, V10).
• Ovarian excision codes already include unilateral or bilateral work; E090 is the one exception, for S745 when the contralateral ovary has moderate or severe endometriosis (V10, note 2).
• Marked #: hospital-funded premises, equipment and staff are not included (V8).
• E090 is on the list of codes that take the E676 morbid obesity premium (SP6).

• S757, S758 or S759 hysterectomy (V8).
• S745 oophorectomy (V10).

Common rejections

• ADF: The base procedure was missing, invalid or paid at zero; this add-on is only payable with S757, S758, S759 or S745 (V8, V10).
