Explanation

Updated October 26, 2026

Vaginal hysterectomy, with or without removal of the tubes and ovaries.

What it covers

S816 is a vaginal hysterectomy, with or without adnexa unless otherwise specified (V8). If anterior and/or posterior vaginal repair is also done, bill S758 or S759 instead (V8).

The Schedule notes that hysterectomy for benign conditions should only be done when less invasive treatments have failed or been declined (V8 commentary).

Billing rules

  • Add E862 (+25%) when done with laparoscopic assistance (V8).
  • E090 (ovaries with endometriosis) is listed only with S757, S758 and S759, not S816 (V8).
  • S722, S760, S812, S813, S738, S741, S745, S747, S780, S781 and S782 are not payable with S757, S816, S758, S759, S710, S763 or S762 (V8, note).
  • A D&C is not payable with a hysterectomy (V1, rule 2).
  • More than one procedure under the same anaesthetic (or within 14 days in the same admission for the same condition): the major procedure is paid in full and the others at 85% (SP2, §3).
  • The surgical fee does not include the major pre-operative visit (where the decision to operate is made), which is billed separately; the surgeon can't bill the hospital admission assessment unless it is that visit (SP2, §1).
  • S816 is not on the list of codes that take the E676 morbid obesity premium (SP6).

Often billed with

  • E862 for laparoscopic assistance (V8).
  • E854 ureterolysis (V1).
