Most responsible physician's visit on the day an LTC patient is discharged after 30+ days, when another provider will take over care.

What it covers

W124 is the subsequent visit on the day of discharge from a chronic care or convalescent hospital, nursing home or home for the aged (A19). It must meet all the elements and rules of the MRP discharge-day visit on GP45, including a discharge summary within 48 hours (A20, GP45).

The discharge summary must include at least the reason for admission, procedures done, discharge diagnosis, discharge medications, and the follow-up required (A20, GP45).

Billing rules

• The patient must have been admitted for at least 30 consecutive days (A20).
• The patient must be discharged to another facility or to home, where ongoing care is expected to be taken over by a different provider (A20).
• Not payable for a patient admitted for respite care (A20).
• Payable only to the MRP, once per admission; not for transfers within the same institution (GP45).

Common rejections

• AO3: 'MRP visit already paid': a discharge-day MRP visit has already been paid for this admission (GP45).
