Visit by the most responsible physician on the day of hospital discharge, including the discharge summary, follow-up and discharge prescriptions.
What it covers
C124 pays the MRP for a subsequent visit on the day of discharge. It also requires a discharge summary completed within 48 hours of discharge, arranging follow-up as appropriate, and prescribing discharge medications if any (GP45).
The discharge summary must include at least the reason for admission, procedures done in hospital, the discharge diagnosis and medications on discharge (GP45).
Billing rules
- Payable only to the MRP, once per hospital admission (GP45).
- Not payable if the patient was discharged within 48 hours of admission, for transfers within the same hospital, or for discharge directly from an ICU or NICU where a critical care per diem was paid that day (GP45).
- Not payable for an obstetrical delivery or newborn admission unless the mother or baby was in an ICU or NICU and later discharged from another unit (GP45).
- Only one of C124 or C126 per admission (A13). C122, C123, C142 and C143 are not payable the same day (GP44, GP46).
- If more than one physician claims it, the physician the patient is rostered to may receive the payment (GP45).
- Add E083 (MRP premium) or, on a Saturday, Sunday or holiday, E084, when you are the MRP; only one per patient per day (GP47).
- Qualifies for the automatic hospitalist premium for eligible FPs and internists with high in-patient volumes (GP103).
- FHO: counts toward the hospital services special payment (Agreement Schedule 3, p.90; §8.5, p.66). In-patient services to enrolled patients are paid full fee-for-service (Agreement p.64 §7.2).
Often billed with
- E083 or E084 MRP premium (GP47).
Common rejections
- AO3: 'MRP visit already paid': C124 or C126 has already been paid for this admission, possibly to another physician (GP45, A13).
