Follow-up colposcopy with one or more biopsies.

What it covers

Z787 is a follow-up colposcopy with biopsy(ies) (V6). The first colposcopic investigation of an abnormal screening test or lower genital tract lesion is Z731; a follow-up without biopsy is Z730 (V6).

Billing rules

  • The biopsies are part of Z787. Z720 (cervical biopsy) is not payable with Z731, Z787 or Z730 (V6, rule 4).
  • Includes collection of cervical cancer screening specimens for cytology and/or endocervical curetting, and for HPV testing when done for screening-related indications (V6, rules 1–2).
  • G365 and G394 (cervical screening specimen collection) are not payable with Z731, Z787 or Z730 (V6, rule 3).
  • A screening colposcopy is included in the assessment and is not billed as a colposcopy code (V6, rule 5).
  • Z-codes are independent operative procedures (IOP). If the major pre-operative visit was in the previous 12 months, the only assessment payable before the IOP that day is a minor assessment (GP) or partial assessment (specialist); if the major pre-operative visit is the same day, no other assessment before the IOP is payable (SP4, §13).
