Explanation
Updated October 26, 2026
Additional cervical cancer screening specimen, only for the specific purposes listed in the Schedule. The indication must be charted.
What it covers
G394 is collection of additional cervical cancer screening specimen(s) for one of these purposes: once every 3 years for immunocompromised patients; once at least 24 months after a positive test for another high-risk HPV subtype with normal or low-grade cytology; once at least 24 months after colposcopy discharge when guidelines recommend increased screening; a repeat after an invalid HPV test or unsatisfactory cytology; post-hysterectomy vault testing when the cervix showed dysplasia; or a patient in a vulnerable group who may have difficulty accessing screening in time (J47).
The follow-up test once after low-grade cytology applies only where the follow-up specimen(s) are collected between March 3, 2025 and September 30, 2026 (J47). Routine screening is G365 (J46).
Billing rules
- The clinical indication must be documented in the medical record, or G394 is not payable (J48).
- Post-hysterectomy vault testing is limited to once per patient per lifetime (J47). Specimens beyond the specified limits are not insured (J48).
- Not payable with a consultation, repeat consultation, general or specific assessment or re-assessment, or routine postnatal visit when a pelvic exam is part of it; add E431 to that visit instead when collected outside hospital or an ICHSC (A4, J47).
- Not payable with an insured colposcopy (Z731, Z787, Z730), though E431 may still be added (J47, V6).
- Marked (+): add G700 when collection is the sole reason for the visit (J3, J47).
- On the list of services payable on the same day as counselling by the same physician (GP58).
- In long-term care, G394 is included in the W010 monthly management fee (GP51). FHO: listed in Schedule 2A (long-term care core services) but not in Schedule 2 (core services), so not part of the base rate basket for other enrolled patients (Agreement p.84, pp.75–82).
Often billed with
- E431 when collected outside hospital or an ICHSC (J47).
- G700 when collection is the only reason for the visit (J3).
- Q011A, the zero-fee Pap smear tracking code used by preventive care bonus models such as FHO and FHN (OMA Q-codes guide pp.7, 18).
Common rejections
- A2B: OHIP describes this as 'Service not usually performed for the patient's sex': the code didn't match the sex on the patient's OHIP record; check the health number and patient details on the claim.
