Extra payment on earlier claim

Explanation
Updated October 26, 2026
OHIP paid more on an earlier claim, usually after reprocessing it. It often comes paired with a 55 deduction; check the net amount.
What it means
Code 57 marks a payment that adjusts an earlier claim. It is not a new claim and not an error. It shows on this month's Remittance Advice (RA) because OHIP has changed what it pays for a service you billed before.
Often a 57 sits beside a 55. When the Ministry reprocesses a claim, it reverses the old payment (55) and pays the reprocessed amount (57). If nothing changed, the two lines cancel out to $0. If the 57 is larger, you were paid more.
A 57 on its own is usually extra money: a Ministry correction, the result of an inquiry you sent, or a change in how the visit is paid. A common example is a patient who left a physician's roster. Visits that were shadow billed (code I2) can later be repaid at the full fee-for-service amount.
Common causes
- Ministry reprocessing after a fee or payment rule change (paired with 55)
- A roster change, for example shadow-billed visits repaid at full fee-for-service
- The outcome of a Remittance Advice Inquiry you sent
- A Ministry correction of an earlier underpayment
How to fix it
- Trace the 57 to the original claim: same patient, service date and fee code.
- Check for a 55 line on the same RA and work out the net change.
- If the net amount is what the service should pay, there is nothing to do.
- If you think the adjusted amount is still too low, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the date of that RA.
Don't resubmit if
- Never send a 57 line as a new claim. It is a payment, not a rejection.
- The 57 and its matching 55 net to the amount you expected.
- You think the adjusted amount is too low. Use an RAI; a new claim would be a duplicate.
How APEX handles it
APEX treats 57 as a code that can't be corrected and keeps it out of the Correction Centre. In claim history, APEX pairs each 55 deduction with the earlier payment it reverses, strikes both through, and highlights the payment still standing, so a 57 that replaced an earlier payment is easy to follow.
