Description
Claim assessed by Assessment Officer

Explanation

Updated October 26, 2026

An OHIP Assessment Officer assessed this claim and set the payment. It's informational unless the amount looks wrong.

What it means

Code 62 means the claim was assessed by an Assessment Officer rather than paid automatically. The amount on the line is the result of that assessment.

How to fix it

  1. Check the amount paid against what you claimed.
  2. If you believe OHIP got it wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with your supporting details.

Don't resubmit if

  • The claim was paid. Sending it again would be a duplicate.
  • The claim is already on the RA. Changes to a claim on the RA go through an RAI, not a new claim.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
