Explanation
Updated October 26, 2026
OHIP paid this elective service at 75% of the OHIP schedule of rates.
What it means
Code 69 means the service was treated as elective and paid at 75% of the OHIP schedule of rates.
The Ministry's list doesn't explain which services or situations this applies to.
How to fix it
- Check that the paid amount is 75% of the Schedule fee.
- If you believe the service wasn't elective, or shouldn't have been reduced, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The claim is already on the RA. Changes to a claim on the RA go through an RAI, not a new claim.
