- 50
- Description
- Paid per the Schedule of Benefits
Explanation
Updated October 26, 2026
OHIP paid this line at the amount the Schedule of Benefits allows. It's a payment note, not a rejection; check only if the amount looks wrong.
What it means
Code 50 means the service was paid in accordance with the Schedule of Benefits. It is an informational code on the Remittance Advice (RA). The claim was approved.
OHIP pays insured services at the amounts set by the Schedule. Code 50 tells you OHIP applied that amount to this line. It usually explains why the amount paid differs from the amount billed.
In most cases nothing needs doing. It's worth a look only when the paid amount is lower than you expected for the service you provided.
Common causes
- The claim billed more than the Schedule fee, for example an old fee or a typing error in the amount
- A premium or percentage add-on was worked out by OHIP from the Schedule, not from the amount billed
- The Schedule fee changed between the service date and the date you set the amount
How to fix it
- Compare the amount paid with the Schedule fee for that code on the service date.
- If they match, there is nothing to fix. Update the fee in your billing system if you billed an old amount.
- If the paid amount is lower than the Schedule fee and no other explanatory code explains it, look at the other codes on the claim. Rules such as D3 or V8 reduce payment and carry their own code.
- If you still believe the payment is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The amount paid matches the Schedule fee. This is the most common case, and there is nothing to correct.
- You billed more than the Schedule fee. OHIP pays the Schedule amount, and a new claim would be a duplicate.
- The claim is on the RA. Questions about a paid claim go through an RAI, not a new claim.
How APEX handles it
APEX has no correction rule for 50, and claims with a payment aren't listed in the Correction Centre. APEX judges the line by its amounts: paid in full counts as Settled. A line paid less than billed is marked Underpaid when other codes on the same claim were paid, and Settled when it is the only code on the claim. See how APEX works · Pricing
