Error code
C3 — Allowed at minor assessment fee
Paid as minor assessment
The assessment you billed was paid at the minor assessment fee instead of the fee code billed.
What to do: Look for a rule that applies, such as a limit or same-day procedure, before sending an inquiry.
- Code
- C3
- Description
- Allowed at minor assessment fee
Explanation
Updated October 26, 2026
OHIP paid this visit at the minor assessment fee instead of the fee code billed. Check the rule that applies before sending an RAI.
What it means
Code C3 means the assessment you billed was paid at the minor assessment fee. A minor assessment is a brief history and exam of the affected part or region, or brief advice or information (GP27).
The Ministry doesn't say which rule caused the reduction. One rule in the Schedule: when the major pre-operative visit for a Z-code procedure was in the previous 12 months, only a minor assessment (family physicians) or partial assessment (specialists) is payable before the procedure on the same day (SP4).
Common causes
- An assessment billed before a Z-code procedure on the same day
- A visit whose documentation supports only a minor assessment
- A limit on the code billed that reduced it to a lesser fee
How to fix it
- Compare the code billed with the minor assessment and look for a rule (limit, same-day procedure) that applies.
- If your chart supports the code you billed and no rule restricted it, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details such as your chart notes.
- Otherwise, the lower payment stands.
Don't resubmit if
- The claim is already on the RA and you only disagree with the decision. Use an RAI, not a new claim.
How APEX handles it
APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
