Description
Allowed as Type 2 admission assessment

Explanation

Updated October 26, 2026

OHIP paid this long-term care admission assessment as a Type 2 admission assessment, because you saw the patient before admission.

What it means

Code C6 means a long-term care admission assessment was paid at the Type 2 fee.

A Type 1 admission assessment is a general assessment on admission. If the physician gave a consultation, general assessment or general re-assessment before the admission, the payment is adjusted to a lesser fee. A Type 2 admission assessment is the admitting physician's first visit after admission when they had already given one of those services before admission (GP49).

Common causes

  • You gave a consultation, general assessment or general re-assessment before the admission

How to fix it

  1. Check whether you billed a consultation, general assessment or general re-assessment for this patient before admission.
  2. If you didn't, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details such as your chart notes.
  3. If you did, the Type 2 payment is correct.

Don't resubmit if

  • You saw the patient for one of those services before admission.
