HA — Admission assessment billed by another physician - paid as hospital visit

Paid as hospital visit

Another physician was already paid the admission assessment for this stay, so your claim was paid at the lower hospital visit fee.

What to do: If you took over care by transfer, nothing to fix: the hospital visit fee is the right payment.

Description
Admission assessment billed by another physician - paid as hospital visit

Explanation

Updated October 26, 2026

Another physician was already paid the admission assessment for this hospital stay, so your claim was paid at a hospital visit fee instead.

What it means

HA means an admission assessment had already been claimed by another physician for this admission, so OHIP paid your claim at the hospital visit fee. The claim was paid, at a lower amount; it was not rejected.

An admission assessment is the initial assessment done to admit a patient to hospital, and the admitting physician is the one who does it (GP40). When an in-patient is transferred from one physician to another, only one consultation, general or specific assessment or re-assessment is payable per admission. An additional admission assessment is not payable when the patient is transferred to another physician within the same hospital (GP40).

This also affects the MRP admission premium. E082 (+30%) is only payable when the admitting physician is the most responsible physician (MRP). If the MRP doesn't render the admission assessment, E082 isn't payable to any physician during that admission (GP42).

Common causes

  • The patient was transferred to you from another physician in the same hospital, who had already billed the admission assessment
  • The emergency physician did the admission assessment. Under GP42 it is payable to them (as C004) even when the patient is admitted under another physician.
  • Two physicians both billed an admission assessment (for example C003A, C004A or C933A) for the same admission

How to fix it

  1. Find out which physician did and billed the admission assessment for this admission.
  2. If you took over care by transfer, the hospital visit fee is the right payment. There is nothing to fix.
  3. If you did the admission assessment and the other physician didn't, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with the admission notes.
  4. For later visits in the admission, bill the subsequent visit codes (for family physicians C002A, C007A or C009A, by week of stay, A13).

Don't resubmit if

  • The claim was paid on the RA. Disputes go through an RAI, not a new claim.
  • The admission assessment was correctly billed by another physician, such as on a transfer of care.

How APEX handles it

APEX has no correction rule for HA and doesn't send inquiries for it. A claim paid at the lower fee is marked Underpaid, or Settled when the assessment is the only fee code on the claim, so you can decide whether an RAI is worth sending. See how APEX works · Pricing
