Error code
D3 — Not allowed in addition to visit fee
Not payable with the visit
This service isn't payable in addition to the visit fee on the claim, so OHIP paid the visit and cut this line.
What to do: Check what was paid for the whole visit; if the rules don't allow both, resubmitting won't change it.
- Code
- D3
- Description
- Not allowed in addition to visit fee
Explanation
Updated October 26, 2026
OHIP won't pay this service on top of the visit fee billed with it, so the line was cut or paid $0. Check the visit total before acting.
What it means
D3 means this service is not payable in addition to the visit (assessment) fee on the claim. OHIP pays the visit and reduces or zeroes the other line. The claim shows on the Remittance Advice (RA) as partly paid; it is not rejected to the error report.
This is a bundling rule. Under the Schedule of Benefits some services are treated as part of the visit, or can't be paid with it on the same day.
Before you accept it, check two things: that the two services really can't be paid together, and that the visit was paid the right amount overall. If the lower-paying code was kept when the rules allowed the higher one, the difference may be recoverable through an inquiry.
Common causes
- A service billed with an assessment when the Schedule treats it as part of the visit
- Primary mental health care (K005A) billed with an assessment on the same visit
- Two codes billed for the same encounter when only one is payable
How to fix it
- Look at what was paid for the whole visit, not just the D3 line.
- Check the Schedule of Benefits payment rules for the codes billed together.
- If the rules don't allow both, there is nothing to fix. Resubmitting won't change the result.
- If you believe the visit was underpaid, for example both services were payable or the higher-paying code should have been paid, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date. Explain the correction you are asking for.
Don't resubmit if
- The claim is on the RA. Changes to a paid claim go through an RAI, not a new claim.
- The Schedule doesn't allow the two services together.
How APEX handles it
APEX usually marks partly paid D3 lines as Underpaid so they stay visible. Its claim adjustment tool flags D3 visits where the highest-priced code wasn't the one paid, and A007A or A003A visits with K005A paid less than the full pair. It prepares these as adjustment requests to the Ministry (an RAI, or a bulk spreadsheet for larger batches) while the RA is within 7 months. See how APEX works · Pricing
