Error code
DD — Diagnostic code unrelated to the original eye exam
Diagnosis unrelated to eye exam
The diagnostic code on this eye service isn't related to the original eye exam, so it wasn't paid.
What to do: Check the diagnostic code on this claim and on the original eye exam.
- Code
- DD
- Description
- Diagnostic code unrelated to the original eye exam
Explanation
Updated October 26, 2026
OHIP didn't pay this eye service because the diagnostic code isn't related to the original eye exam. Check the diagnostic code.
What it means
Code DD means the service wasn't allowed because its diagnostic code is unrelated to the original eye exam.
The periodic oculo-visual assessment is limited to one per patient per 12 months (ages 19 and under, or 65 and over), and any other service by the same physician (other than an ophthalmologist) on the same day isn't payable. A re-assessment after a periodic oculo-visual assessment is claimed with a lesser assessment fee and diagnostic code 367 (GP28).
Common causes
- A follow-up eye assessment billed with a diagnostic code other than 367
- A diagnostic code that doesn't match the eye exam it follows
How to fix it
- Check the diagnostic code on the claim and on the original eye exam.
- If the follow-up was a re-assessment after the oculo-visual assessment and the diagnostic code was wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details such as your chart notes.
- Otherwise, the refusal stands.
Don't resubmit if
- The claim is already on the RA and you only disagree with the decision. Use an RAI, not a new claim.
