Explanation

Updated October 26, 2026

Code VB is about the extra eye exam (OVA) allowed once in the second year for patients aged 20 to 64 after a periodic OVA. Check the exam limits.

What it means

The Ministry describes VB as: an additional oculo-visual assessment (OVA) is allowed once within the second year for patients aged 20 to 64, following a periodic OVA.

In the physician Schedule, periodic OVAs (A110, A112) are insured only for patients aged 19 or less or 65 or more, one per 12 months (GP28). For patients aged 20 to 64, the eye exams payable on behalf of ODSP and Ontario Works (K065, K066, Appendix F) are limited to one every 24 months. A major eye examination (A115) for patients 20 to 64 needs a listed medical condition or a requisition (A10–A11).

Common causes

  • A second eye exam for a patient aged 20 to 64 within the limit period

How to fix it

  1. Check the patient's age and earlier eye exams by any physician or optometrist.
  2. If the exam was over the limit, there is nothing to fix.
  3. If you believe the payment is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.

Don't resubmit if

  • You disagree with how a claim already on the RA was paid. Use an RAI, not a new claim.
