Code
DE
Description
Lab tests already paid - visit fee adjusted

Explanation

Updated October 26, 2026

OHIP reduced the visit fee because lab tests for the patient were already paid. Check what the visit and the tests covered.

What it means

Code DE means lab tests for the patient were already paid, and the visit fee was adjusted because of them.

The Ministry doesn't say which tests or visits this applies to.

How to fix it

  1. Find the paid lab tests and compare them with the visit.
  2. If the visit was separate from the tests, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details such as your chart notes.
  3. Otherwise, the adjusted payment is correct.

Don't resubmit if

  • The claim is already on the RA and you only disagree with the decision. Use an RAI, not a new claim.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
