Error code
E6 — Service date after eligibility ended - patient deceased
Recorded as deceased
The Ministry's records show the patient died before the service date, so coverage had ended.
What to do: Check the service date and health number; if the patient is alive, the Ministry's record needs correcting.
- Code
- E6
- Description
- Service date after eligibility ended - patient deceased
Explanation
Updated October 26, 2026
E6 means the Ministry's records show the patient died before the service date. Check the service date and health number.
What it means
E6 appears on the Remittance Advice (RA) when the service date is after the patient's eligibility ended, and the Ministry ended it because its records show the patient is deceased.
If you saw the patient on that date, something is wrong: either the claim details or the Ministry's record.
Common causes
- The service date on the claim is after the patient's date of death
- The claim carries another person's health number
- The Ministry's record of the patient's death is wrong
How to fix it
- Check the service date and health number on the claim against the chart.
- If the claim details were wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with the correct details.
- If the patient is alive and the record is wrong, ask them to contact ServiceOntario to correct it, then follow up with an RAI.
Don't resubmit if
- The service date really was after the patient's death.
- The claim is already on your RA. A new claim for the same service will be treated as a duplicate. Use an RAI instead.
How APEX handles it
APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
