Error code
EH6 — Eligibility ended - patient deceased
Recorded as deceased
The Ministry's records show the patient died before the service date, so their eligibility had ended.
What to do: Check the service date and health number against the chart; correct and resubmit if either was wrong.
- Code
- EH6
- Description
- Eligibility ended - patient deceased
Explanation
Updated October 26, 2026
EH6 means the Ministry shows the patient as deceased before the service date. Check the date and health number on the claim.
What it means
EH6 means the patient's eligibility was terminated because the Ministry's records show the patient is deceased, and the service date is after that. The claim was rejected.
The matching Remittance Advice code is E6.
Common causes
- The service date is after the patient's date of death
- The claim carries another person's health number
- The Ministry's record of the death is wrong
How to fix it
- Check the service date and health number against the chart.
- If the claim details were wrong, correct them and resubmit. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.
- If the patient is alive and the record is wrong, ask them to contact ServiceOntario. Resubmit after the record is corrected.
Don't resubmit if
- The service date really was after the patient's death.
