EB — Coding added/changed per the Schedule of Benefits

Coding changed by OHIP

OHIP added or changed the coding on your claim to follow the Schedule of Benefits; the amount paid may differ.

What to do: Compare the code and amount on the remittance advice with what you submitted, and update your billing if needed.

Code
EB
Description
Coding added/changed per the Schedule of Benefits

Explanation

Updated October 26, 2026

EB means OHIP added or changed coding on your claim to match the Schedule of Benefits. Check what changed and the amount paid.

What it means

EB appears on the Remittance Advice (RA) when OHIP added to or changed the coding on your claim so that it follows the Schedule of Benefits. The line was processed under the corrected coding.

It is an explanation, not a rejection. The amount paid may be higher, lower or the same as what you billed.

How to fix it

  1. Compare the code and amount on the RA with what you submitted.
  2. If the change is right, update your billing so future claims use the correct coding.
  3. If you disagree, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.

Don't resubmit if

  • The claim is already on your RA. A new claim for the same service will be treated as a duplicate. Use an RAI instead.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
