EF5 — ICHSC practitioner 991000 cannot bill insured services

Insured service under 991000

Insured services were billed under the ICHSC practitioner number 991000, which can't bill insured services.

What to do: Bill the insured service under the billing number of the physician who provided it; keep only ICHSC fees under 991000.

Code
EF5
Description
ICHSC practitioner 991000 cannot bill insured services

Explanation

Updated October 26, 2026

EF5 means insured services were billed under ICHSC practitioner number 991000, which isn't allowed. Bill them under a physician's number.

What it means

EF5 means the claim used the Integrated Community Health Services Centre (ICHSC) practitioner number 991000 to bill insured services. That number is not allowed to bill insured services, so the claim was rejected.

In the Ministry's error list, specialty code 90 must be used with provider number 991000 (see V08).

Common causes

  • An insured physician service was billed under 991000 instead of the physician's own billing number

How to fix it

  1. Bill the insured service under the billing number of the physician who provided it.
  2. Keep only ICHSC fees under 991000.
  3. Resubmit. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.

Don't resubmit if

  • Nothing has changed. Sending the same claim again will reject again.
