- Code
- EF3
- Description
- Insured services are excluded from ICHSC billings
Explanation
Updated October 26, 2026
EF3 means an insured service was billed on an ICHSC claim, which isn't allowed. Bill insured physician services separately.
What it means
EF3 means the claim billed under an Integrated Community Health Services Centre (ICHSC) included insured services, which are excluded from ICHSC billings. The claim was rejected.
The Ministry's error list also says the ICHSC practitioner number 991000 can't bill insured services (EF5).
Common causes
- A physician's insured service was put on the facility's claim
- The claim was submitted under the facility's number instead of the physician's billing number
How to fix it
- Remove the insured service from the ICHSC claim.
- Bill the insured service under the physician's own billing number.
- Resubmit. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.
Don't resubmit if
- Nothing has changed. Sending the same claim again will reject again.
