Error code

EP5 — Incorrect FSC for Group Type

Code not for your model

The fee code isn't valid for your group's model type; many primary care Q-codes are model-specific.

What to do: Replace the code with your enrolment model's version, or fix the group number, and resubmit.

Code
EP5
Description
Incorrect FSC for Group Type

Explanation

Updated October 26, 2026

EP5 means this fee code isn't valid for your group's model type. Use the code that belongs to your patient enrolment model.

What it means

EP5 means the fee schedule code billed is not valid for the type of group on the claim, so the claim was rejected.

Many primary care Q-codes are model-specific. For example, the after-hours premium is Q012A for FHG, FHN, FHO and several other models, but Q016A for the Comprehensive Care Model (CCM) (Schedule page AQ2).

Common causes

  • A Q-code for a different enrolment model was billed
  • The claim was submitted under the wrong group number

How to fix it

  1. Check which models can bill the code in Schedule Appendix Q.
  2. Check the group number on the claim.
  3. Replace the code with your model's version, or fix the group number, and resubmit. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.

Don't resubmit if

  • Your model has no version of this code.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and, for many claims rejected with this code, works out the fix for you to approve before it is resubmitted. See how APEX works · Pricing
