Error code
EQJ — Provider not eligible for this fee on service date
Wrong new patient fee
You weren't eligible for this new patient fee: Q013 and the new graduate fee Q033 were mixed up.
What to do: Check which new patient fee your registration allowed on the service date and resubmit with that code.
- Code
- EQJ
- Description
- Provider not eligible for this fee on service date
Explanation
Updated October 26, 2026
EQJ means you weren't eligible for this new patient fee on the service date: the new graduate and regular new patient fees were mixed up.
What it means
EQJ means the provider was not eligible for the fee on the date of service. The Ministry's example: a new graduate billed the regular new patient fee (Q013), or a physician who is not a new graduate billed the new graduate new patient fee (Q033).
Common causes
- A new graduate billed Q013 instead of Q033
- A physician who is not a new graduate billed Q033
How to fix it
- Check which new patient fee your registration allowed on the service date.
- Resubmit with the fee code that matches your status, if that fee code is still in effect on the service date.
- Resubmit the corrected claim within three months of the service date. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.
Don't resubmit if
- The fee code is not in effect on the service date.
