Code
EQN
Description
Registration Usage Error on Service Date

Explanation

Updated October 26, 2026

EQN means your registration for this service wasn't in effect on the service date. Check the dates of your registration.

What it means

EQN (Registration Usage Error on Service Date) means you have a registration for the service, but it did not cover the date of service on the claim. The Ministry gives no further detail for this code.

Common causes

  • The service was given before the registration started or after it ended
  • The service date on the claim is wrong

How to fix it

  1. Check the service date on the claim.
  2. Check the start and end dates of the registration with the Ministry.
  3. If the date was wrong, correct it and resubmit. If the registration should have covered the date, ask the Ministry to correct it first.
  4. Resubmit the corrected claim within three months of the service date. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.

Don't resubmit if

  • The service date falls outside your registration.
