Explanation
Updated October 26, 2026
H4: after the 6th week in hospital, a weekly maximum applies to paediatricians' visits. Visits over the limit weren't paid.
What it means
H4 means OHIP applied a weekly maximum to a paediatrician's hospital visits after the 6th week of the stay, and visits over the maximum were not paid.
How to fix it
- Count the visits billed for that week and check the admission date on the claims.
- If you believe the maximum was applied in error, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The claim is already on your RA. Use an RAI, not a new claim, if you disagree.
