Error code
HB — Subsequent visit already paid on same day
Hospital visit already paid
A subsequent hospital visit was already paid for this patient that day, so this one wasn't paid.
What to do: Check whether the visit was billed twice; for a real second assessment, check whether C121 or a special visit applies.
- Code
- HB
- Description
- Subsequent visit already paid on same day
Explanation
Updated October 26, 2026
HB: a subsequent hospital visit was already paid for this patient that day, so this one wasn't paid. Use C121 for acute intercurrent illness.
What it means
HB means OHIP already paid a subsequent visit for this patient on the same day, so this one was not paid.
In hospital, subsequent visits are limited to one per patient per day for the first 5 weeks after admission. After 5 weeks, visits for an acute intercurrent illness over the limits are billed as C121 (Schedule GP43).
Common causes
- Two subsequent visits billed for the same patient on the same day
- A duplicate claim
How to fix it
- Check whether the same visit was billed twice.
- If the patient really needed a second assessment, check whether another code applies (for example, C121 after week 5, or a special visit).
- If you believe the claim should have been paid, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The claim is already on your RA. Use an RAI, not a new claim, if you disagree.
