Error code
H7 — Allowed as chronic care
Paid as chronic care
The visit was paid as chronic care, not at the code you billed, based on the patient's bed or institution type.
What to do: Check the bed type on the visit date; if it was acute care, send a remittance advice inquiry.
- Code
- H7
- Description
- Allowed as chronic care
Explanation
Updated October 26, 2026
H7: the visit was paid as chronic care, not at the code you billed. Check the type of bed or institution the patient was in.
What it means
H7 means OHIP paid the visit as chronic care instead of the code you billed.
Visits to patients in chronic care hospitals, convalescent hospitals and designated chronic or convalescent care beds in hospitals are billed with W-prefix codes, which have their own monthly limits (Schedule GP49, A19).
How to fix it
- Check the type of bed the patient was in on the visit date.
- If the patient was in an acute care bed, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
- For future visits to chronic care patients, bill the W-prefix codes.
Don't resubmit if
- The claim is already on your RA. Use an RAI, not a new claim, if you disagree.
How APEX handles it
APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
