- Code
- H2
- Description
- Initial visit already billed - paid as subsequent visit
Explanation
Updated October 26, 2026
H2: an initial visit was already claimed for this patient, so this one was paid as a subsequent visit. Usually correct.
What it means
H2 means OHIP paid the visit as a subsequent visit because an initial visit (such as the admission assessment) had already been claimed. In hospital, a subsequent visit is any routine assessment after the admission assessment (Schedule GP43).
Common causes
- An admission or initial assessment billed after the patient's admission assessment was already paid
- The patient was transferred within the same hospital
How to fix it
- Check whether an initial visit was already paid for this admission.
- If yours was the true initial visit, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
- Otherwise, the subsequent visit payment is correct.
Don't resubmit if
- The claim is already on your RA. Use an RAI, not a new claim, if you disagree.
