Error code

J1 — Service date is before OHIP coverage started

Before coverage started

The service date is before the patient's OHIP coverage started, so OHIP doesn't pay it.

What to do: Check the health number and service date, run a health card validation, or have the patient contact ServiceOntario.

Code
J1
Description
Service date is before OHIP coverage started

Explanation

Updated October 26, 2026

J1 means the service date is before the patient's OHIP coverage started. Check the health number and date, or have the patient contact ServiceOntario.

What it means

J1 means the Ministry's records show the patient's OHIP coverage started after the date of service. OHIP does not pay for services before coverage begins.

It matches the Error Report code EH1 (service date before eligibility effective date), but it appears on the Remittance Advice.

Common causes

  • The patient was new to Ontario or newly registered and was seen before coverage started
  • The wrong health number was used (for example a family member's)
  • The service date was entered wrongly

How to fix it

  1. Check the health number and service date on the claim against the patient's card and chart.
  2. Run a health card validation check to see the patient's coverage dates.
  3. If the patient believes they were covered on that date, advise them to contact ServiceOntario about their eligibility.
  4. If you believe the decision is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.

Don't resubmit if

  • The patient had no OHIP coverage on the service date.
  • The line is already on your RA. Question it with an RAI, not a new claim.

How APEX handles it
