Error code

J2 — Service date is after OHIP coverage ended

After coverage ended

The service date is after the patient's OHIP coverage ended, so OHIP doesn't pay it.

What to do: Check the health number and service date, run a health card validation, or have the patient contact ServiceOntario.

Code
J2
Description
Service date is after OHIP coverage ended

Explanation

Updated October 26, 2026

J2 means the service date is after the patient's OHIP coverage ended. Check the health number, or have the patient contact ServiceOntario.

What it means

J2 means the Ministry's records show the patient's OHIP coverage had ended before the date of service. OHIP does not pay for services after coverage ends.

It matches the Error Report code EH4 (service date after eligibility end date), but it appears on the Remittance Advice.

Common causes

  • The patient moved out of Ontario or lost eligibility
  • Coverage ended because the patient did not respond to a notice to register (see E9)
  • The Ministry's records show the patient as deceased (see E6)
  • The wrong health number or service date was used

How to fix it

  1. Check the health number and service date on the claim against the patient's card and chart.
  2. Run a health card validation check to see the patient's current coverage status.
  3. If the patient believes they were covered, advise them to contact ServiceOntario about their eligibility.
  4. If you believe the decision is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.

Don't resubmit if

  • The patient had no OHIP coverage on the service date.
  • The line is already on your RA. Question it with an RAI, not a new claim.

How APEX handles it
