Error code
J7 — Claim submitted more than 3 months after service date
Submitted too late
The claim reached OHIP more than three months after the service date, so it's stale-dated and normally not paid.
What to do: If it was first submitted on time and rejected, use the Ministry's stale-dated resubmission process.
- Code
- J7
- Description
- Claim submitted more than 3 months after service date
Explanation
Updated October 26, 2026
J7 means the claim reached OHIP more than three months after the service date. Use the stale-dated process only if it was first sent on time.
What it means
Claims must reach OHIP within three months of the service date. J7 flags a claim submitted after that. A claim that is past the three-month limit is stale-dated and is normally not paid.
There are exceptions, for example when the claim was first submitted on time and rejected, or when the delay was outside your control. These go through the Ministry's stale-dated process.
Common causes
- The claim was held, lost or never sent
- A rejected claim was corrected and resubmitted after the three-month window
- The service date was entered wrongly, making the claim look older than it is
How to fix it
- Check the service date on the claim. If it was a typing error, ask the Ministry to correct it through an RAI.
- If the claim was first submitted on time and later rejected, use the stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
- If the claim was never submitted on time and no exception applies, it can't be paid.
Don't resubmit if
- The claim was first submitted more than three months after the service and no exception applies.
- The line is already on your RA. Question it with an RAI, not a new claim.
How APEX handles it
