- Code
- P2
- Description
- Maximum low birth weight care fee reached
Explanation
Updated October 26, 2026
P2 means the maximum payable for low birth weight care has been reached for this baby. Check earlier paid claims before sending an RAI.
What it means
Code P2 means OHIP reduced or refused this line because the maximum fee allowed for low birth weight care has already been paid for this patient.
The Ministry's description does not say which fee codes or which maximum apply. In the July 2026 Schedule of Benefits, the only listing that names low birth weight is neonatal low birth weight intensive care (G604), payable in place of G600 or G603 for a newborn under 750 grams or 26 weeks gestational age (J33).
Common causes
- Low birth weight care billed more often than the maximum allows
- Another physician on the team already billed the same care for the same days
How to fix it
- Look up earlier paid claims for the same baby and compare the codes and dates.
- If the maximum was reached, no more is payable for this line.
- If you think the line was refused in error, send an RAI with the dates and the care provided. Send it through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The claim is already on your Remittance Advice. Use an RAI, not a new claim.
How APEX handles it
APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
