- Code
- P8
- Description
- Health Care Connect referral older than 3 months
Explanation
Updated October 26, 2026
P8 means the patient was enrolled more than 3 months after their Health Care Connect referral, so the HCC new-patient fee isn't payable.
What it means
Code P8 means OHIP did not pay the Health Care Connect (HCC) new patient fee because more than three months passed between the patient's HCC referral and their enrolment with you.
The complex/vulnerable new patient fee (Q053A) is a one-time payment for enrolling a patient whom HCC has registered as complex/vulnerable. The patient must be enrolled with the physician within three months of being registered with HCC (Appendix Q, AQ2; OMA Q-codes guide).
Common causes
- The enrolment (E/C) form was signed more than three months after the HCC referral
- The service date on the claim doesn't match the date the patient was enrolled
How to fix it
- Compare the HCC referral date with the date on the patient's enrolment and consent form.
- If enrolment really was more than three months later, the fee is not payable.
- If the dates on the claim were wrong, send an RAI with the correct enrolment date and the HCC referral details. Send it through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- Enrolment happened more than three months after the HCC referral.
- The claim is already on your Remittance Advice. Use an RAI, not a new claim.
