PM — Minimum roster size not met

Roster below minimum

Your roster was below the minimum size needed for this payment, such as 650 patients for FHG/CCM preventive care bonuses.

What to do: Check your enrolled patient count on the date that applies; if below the minimum, the payment isn't available.

Code
PM
Description
Minimum roster size not met

Explanation

Updated October 26, 2026

PM means your roster was below the minimum size needed for this payment, such as the FHG/CCM preventive care bonuses (650 patients).

What it means

Code PM means OHIP did not pay this line because the minimum roster size was not met.

The Ministry's description does not name the payments involved. One clear case is the preventive care bonuses (Q100A to Q123A): Family Health Group and Comprehensive Care Model physicians must have at least 650 enrolled patients on March 31 of the bonus year, or 450 for new graduates in their first year (OMA Q-codes guide; Appendix Q).

Common causes

  • A preventive care bonus code billed by an FHG or CCM physician with fewer enrolled patients than the minimum
  • Patients were de-enrolled or not yet enrolled on the count date

How to fix it

  1. Check your enrolled patient count on the date that applies to the payment (for preventive care bonuses, March 31 of the fiscal year).
  2. If the roster was below the minimum, the payment is not available for that period.
  3. If you believe your roster met the minimum, send an RAI with your roster count. Send it through eSubmit, or on form 0918-84, within 7 months of the RA date.

Don't resubmit if

  • Your roster was below the minimum on the count date.
  • The claim is already on your Remittance Advice. Use an RAI, not a new claim.

How APEX handles it
