- Claim Header 2 missing (RMB)
Explanation
Updated October 26, 2026
R09 means a Reciprocal Medical Billing claim was sent without its second claim header (the patient record). Rebuild the claim as RMB.
What it means
Error R09 means the Reciprocal Medical Billing (RMB) claim is missing Claim Header 2.
Reciprocal Medical Billing (RMB) is how Ontario physicians bill OHIP for insured services to patients covered by another Canadian province's or territory's health plan, except Quebec. An RMB claim uses payment program RMB and carries the patient's out-of-province registration details instead of an Ontario health number. RMB claims are the same as regular claims except for the patient information, which is carried in a second claim header record.
Common causes
- The billing software sent payment program RMB but didn't build the RMB patient record
- The out-of-province patient details were missing, so the record was left out
How to fix it
- Check that the patient is set up in your billing software as out-of-province, with their province code and registration number.
- Check the RMB patient fields against the patient's card: the two-letter province code, the registration number from the home province, date of birth, surname, first name and sex. The payment program must be RMB and the payee must be P (pay provider).
- Rebuild and resubmit the claim as RMB. If it rejects again, ask your billing software vendor to check the RMB claim format. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.
