- Code
- R2
- Description
- Up to 10 well-baby visits allowed before age 2
Explanation
Updated October 26, 2026
R2 means the limit of 10 well-baby visits before age two has been reached. Further visits need an assessment for a specific problem.
What it means
Code R2 means OHIP refused this well-baby visit because the patient has already had the maximum of 10 well-baby visits allowed up to two years of age.
Well baby care is a periodic assessment of a well newborn or infant during the first two years of life, with a complete examination, weight and measurements, and advice to the parents (GP34).
Common causes
- More than 10 well-baby visits billed for the child before age two
- Visits for an illness billed as well-baby care
How to fix it
- Count the well-baby visits already paid for this child.
- If the limit was reached, the extra well-baby visit is not payable.
- If the visit was for an illness or concern, it wasn't well-baby care. Send an RAI with the reason for the visit. Send it through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The visit was a routine well-baby check after 10 had been paid.
- The claim is already on your Remittance Advice. Use an RAI, not a new claim.
