Explanation
Updated October 26, 2026
P6 means the child was past the age for well-baby care, which covers the first two years of life. Bill an age-appropriate visit instead.
What it means
Code P6 means OHIP's records show the patient was too old for well-baby care on the service date.
The Schedule of Benefits defines well baby care as a periodic assessment of a well newborn or infant during the first two years of life (GP34). The enhanced 18 month well baby visit (A002, or A268 for paediatrics) is for a child 17 to 24 months old. After the second birthday, a check-up of a well child is a periodic health visit (GP21).
Common causes
- A well-baby visit billed after the child's second birthday
- An enhanced 18 month well baby visit billed after 24 months
- A wrong date of birth on the claim or on the Ministry's record
How to fix it
- Check the child's date of birth and age on the service date.
- If the date of birth was wrong, correct it in the chart and send an RAI with the correct information. Send it through eSubmit, or on form 0918-84, within 7 months of the RA date.
- If the child was over age, bill future well-child check-ups as a periodic health visit for a child (K017, or K267 for paediatrics), which applies after the second birthday.
Don't resubmit if
- The child was over two years old (or over 24 months for the enhanced 18 month visit) on the service date.
- The claim is already on your Remittance Advice. Use an RAI, not a new claim.
