- Team/assist fee already billed
Explanation
Updated October 26, 2026
SD means the team or assistant fee for this procedure was already claimed, so this claim wasn't paid.
What it means
Code SD means OHIP refused this line because a team fee or assistant fee for the same procedure has already been claimed.
The Surgical Preamble says that when two surgeons work together and no team fee is listed, one claims as operating surgeon and the other claims the assistant's fee (SP3, paragraphs 7–8). The General Preamble says that when more than one physician renders parts of a listed service, only one fee is payable unless the Schedule allows otherwise (GP9).
Common causes
- Two physicians both billed the assistant or team fee for the same procedure
- The same assistant claim was sent twice
How to fix it
- Check who was paid the team or assistant fee for this procedure.
- If it is a duplicate, nothing more is payable.
- If a second assistant was needed, note that a second assistant needs medical consultant authorization and a letter from the surgeon, except for the major procedures listed on GP90. Send an RAI with the supporting documents. Send it through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The claim is already on your Remittance Advice. Use an RAI, not a new claim.
