Error code
35 — Duplicate - this service was already claimed
Already submitted
OHIP already has a claim for this patient with the same fee code and service date, so this one wasn't paid.
What to do: Find the first claim on your remittance advice; if it was paid, don't resubmit.
- Code
- 35
- Description
- Duplicate - this service was already claimed
Explanation
Updated October 26, 2026
OHIP already has a claim for this patient, service and date, so this one wasn't paid. Find the original before doing anything.
What it means
Code 35 means OHIP's records show this same service was already claimed for this patient on this date, so the new claim is not paid. The Ministry notes it is used for duplicate claims paid to the practitioner.
In most cases it is a true duplicate. The same visit went in twice, for example when a claim was resent before the first one appeared on a Remittance Advice (RA), or when a 'correction' was sent for a claim that had in fact been paid.
Related codes cover other situations: 32 means services on that day were claimed before, and 36 means another practitioner, group or lab billed the service.
Common causes
- The same claim submitted twice, by mistake or from two billing systems
- A claim resent before the original appeared on an RA
- A corrected claim sent for a visit that was already paid
- Two genuine services with the same code on the same day, billed without supporting information
How to fix it
- Search your billing history for the same patient, service date and fee code.
- If the original was paid, nothing is owed. Do not send the claim again.
- If you can't find a paid original, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, and ask which claim OHIP matched it to.
- If you really provided the same service twice that day at different times, send an RAI with documentation of both. For future claims like this, attach the 'Claims Flagged for Manual Review' form (2404-84); it lists 'duplicate service code claimed for the same date, different time' as a reason to use it.
Don't resubmit if
- The original claim was paid.
- You haven't found the original yet. Sending it again will usually just return 35 again.
- You want to dispute the 35. Use an RAI, not a new claim.
How APEX handles it
APEX treats 35 as a code that can't be corrected and keeps it out of the Correction Centre queue, so it never resubmits these claims. Claim history lists every report line for the patient and service date, newest first, which helps you find the original. See how APEX works · Pricing
