V22 — Invalid Diagnostic Code

Invalid diagnostic code

The diagnostic code on the claim isn't on the Ministry's list; a four-digit code where three digits are needed is a common cause.

What to do: Replace it with the matching three-digit code from the Ministry's list, such as 715 rather than 7159, and resubmit.

V22
Description
Invalid Diagnostic Code

Explanation

Updated October 26, 2026

V22 means the diagnostic code on the claim is not a valid OHIP diagnostic code. Replace it with a code from the Ministry's list and resubmit.

What it means

OHIP accepts only the diagnostic codes in the Ministry's published Diagnostic Codes list. V22 means the code on the claim is not one OHIP accepts.

Most OHIP diagnostic codes are three digits, such as 715 for osteoarthritis. The Ministry's list also has a supplementary set of four-digit codes (for example 7159). In APEX's experience, a four-digit code where the service needs the three-digit code is a common cause of V22 on physician claims.

Common causes

  • A typing error that produced a number not on the list.
  • A four-digit or extended code instead of the three-digit OHIP code for the same condition.
  • A code copied from another coding system or an old code list.

How to fix it

  1. Look up the condition in the Ministry's current Diagnostic Codes list.
  2. Replace the code with the matching OHIP code. If you used a four-digit code, use the three-digit code for the same condition (for example 715 rather than 7159).
  3. Check that the diagnosis is one the fee code allows. Some fee codes accept only certain diagnoses.
  4. Resubmit the corrected claim. If more than three months have passed since the service date and the original claim was sent on time, use the stale-dated claim process described under VJ7.

Don't resubmit if

  • The diagnostic code hasn't changed. It will reject again.

How APEX handles it

When the claim has a four-digit diagnostic code APEX can map (for example 7159 becomes 715), APEX swaps in the three-digit code and keeps the rest of the claim as billed. Any other V22 claim is listed for you to enter a valid diagnostic code before it is resubmitted. See how APEX works · Pricing
