- Code
- V30
- Description
- Fee code and diagnostic code combination not covered
Explanation
Updated October 26, 2026
OHIP doesn't pay this fee code with this diagnostic code. Check both codes against the chart and resubmit if one is wrong.
What it means
V30 ("FSC/Diagnostic Code Combination Not A Benefit (NAB)") means the fee code and diagnostic code on the claim are a combination OHIP doesn't pay.
Some fee codes are only insured for certain diagnoses, or never for some. Some fee code listings in the Schedule name the diagnoses they can be billed with.
Common causes
- The wrong diagnostic code was entered
- The wrong fee code was entered
- The service isn't insured for that diagnosis
How to fix it
- Check the diagnosis in the chart and the fee code's listing in the Schedule.
- If either code was wrong, correct it.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- Both codes are right. The service isn't an OHIP benefit for that diagnosis.
- You haven't changed anything. Sending the same claim again will reject the same way.
How APEX handles it
APEX reads your error reports and remittance advice from MCEDT and, for many claims rejected with this code, works out the fix for you to approve before it is resubmitted. See how APEX works · Pricing
